Submit a Specified Accommodation questionnaire for landlords
Please complete the on-line form by clicking on the blue start button below if you are a landlord who is one of the following:
- Non-metropolitan county council in England
- Housing Association
- Registered charity
- Voluntary organisation
where the tenant is admitted to the accommodation to receive care, support or supervision.
A Local Authority can also apply if they provide a building that is a Domestic Violence Refuge or a Hostel.
Please complete the questionnaire with as much detail as possible.

- Your registration number, company registration number or charitable registration number
- Details of the property such as room numbers
- Full breakdown of rent, including all service charges
- Evidence of policies and procedures requested throughout the questionnaire
Once we have received the questionnaire, a decision will be made as to whether we consider it meets the criteria to be classed as specified accommodation.
