Submit a Specified Accommodation questionnaire for tenants

Please complete the on-line form by clicking on the blue start button below if you think the tenant should be treated as occupying specified accommodation for Housing Benefit purposes. The tenant will be admitted to the accommodation to receive care, support or supervision and the landlord is one of the following:

• Non-metropolitan county council in England
• Housing Association
• Registered charity
• Voluntary organisation
• A Local Authority that provides a building that is a Domestic Violence Refuge or a Hostel.

Please complete the questionnaire with as much detail as possible.
